The Obvious Things
So many times, we expect the cause of a problem is lurking in something complicated. But consider how often the root cause can be traced to:
Fatigue.
Failing to be candid with one another.
Taking on too much.
Rushing.
Not asking for help.
Being reluctant to ask questions.
Making assumptions.
Playing it cool.
Not doing our homework.
Lack of coordination.
