Drift
I met with some executives a while back who said that one part of their operation was "strapped" and overloaded with work.
Being familiar with the organization's history, I asked them further questions and learned that the section which they described as overloaded had five employees doing the same job with the same workload that one employee had done earlier and, by the way, that one employee had performed the job quite well.
They were unaware of the previous staffing and workload. So was the department head.
Unless carefully monitored, things can easily drift.
