Excellent list. You have a fertile imagination and unique gift for seeing the big picture from many angles.

Writing now from the safety of retirement, I remember having seen all the self-destructive choices on your list played out at one time or another. A time or two it was I, unfortunately, who got it wrong.

By way of defence, I also recall a time when in a burst of energy I took a year's subscription the the Harvard Business Review (which for me was like buying a high end set of golf clubs when you not only have a visual disability but hate golf to begin with). One of the classic monographs was entitled "Good Managers Don't Make Policy Decisions." It helped me remain centered on my job, but also liberated me to leave the sinking ship in time to move on to other places while I still had something left to contribute.

It is amazing how instructive our mistakes can be. I know I can recall plenty of mine.

Your point about the HBR reminds me of how often I've read a long book and have emerged with only one idea, but it was a pretty good idea!
