Incremental Disaster

Just as an accumulation of smaller medical conditions may combine to create a major impairment, a series of flawed leaders can eventually produce disaster.

Each of those characters departed amid at least muted cheers, fine dinners, and whatever the symbolic equivalent is of a gold watch and it was only later that their "fumbles" - to use a kind word - were widely acknowledged.

If they are recognized at all. I've seen organizations where a series of CEOs adopted and continued practices that were seriously flawed. As a result, the standards were altered so substandard practices became acceptable if not commendable. The cultural "doctrine" was rewritten on a particular issue. What had been "D" or "F" performance was now an "A" or "B."

Once those assumptions are locked in, serious effort is needed to pull them out.
