The danger of the "We've always done it that way" attitude is not just that it is used to squelch necessary changes but also that it can blind us to the need for any change in the first place. It can lead to an "invisible gorilla in the room" which is not discussed because no one sees it.
Occasionally reviewing the way things have always been done in order to see if there are hidden and harmful assumptions which are adversely affecting results can make enormous sense.
